Qualified employer for Public Service Loan Forgiveness and student loan repayment with the National Health Service Corps. The Nurse Practitioner will provide direct medication management services to patients, maintain case records, and coordinate facility operations and client needs with other staff.
Minimum education requirements include High School Equivalence / GED. The Case Manager provides general case management, plan educational programs, collaborate with staff for medical care, assist with aftercare and continuing care plans, facilitate groups, and coordinate volunteer and educational opportunities.
